Can you try setting set_entvar(grenade, var_dmgtime, 0.0); to set_entvar(grenade, var_dmgtime, 7.0); for example?

Line 832 and 446 like this, test this. In CSGO the molotov damages you for 7 seconds thefore its logical to set var_dmgtime from 0 seconds (not giving you damage) to 7 seconds like it should be. This is just a theory and i'm not a scripter but try it out.

You can try extracting the molotov from here https://dev-cs.ru/resources/942/ or you can wait for someone more knowledgable to respond :P

Yes, I saw that plugin, I don't like it, the fire is not beautifully made
